Form 107 related query

Can someone tell me? How do I submit the following documents to RO :

Detailed Daily work diary (date wise) with job descripttion signed by principal and student. 4) Day wise attendance register signed by student and Principal.5) Request of Condonation letter signed by principal and student On receipt of the above documents further action on the matter will be taken.

In my firm i have never maintained a work diary or attendance register. Please guide how do i make these documents?
