For retotaling a query

Shall i write only roll number, paper to be revalued, exam written, address for communication and enclose a dd that is enough

or i haveto write an application that i am of so and so roll number and need to retotal my paper

please tel;l me which is correct

thanks for your reply but where we should give address for communication etc. with this application is it enough if we attach the dd
