n write ur name  n regstration no. n fexam form no,etc on  d backside of DD..it wud br given in d form in d instructions....

I ALSO FACED THE SIMILAR PROBLEM FOR MY CA FINAL EXAM DD,

I HAVE TAKEN DD BY THE NAME "THE SECRETARY, THE ICAI"(INSTEAD OF FULL FORM)

AND OFCOURSE PAYABLE BRANCH AT DELHI.

BUT I MENTIONED THE REQUIRED DETAILS ON THE BACK SIDE OF DD.

I DONT THINK  ITS A PROBLEM.
