It it very much available in Files portal. Kindly download it. Fixed asset register is mandatory for Companies act , 1956. Income tax act there is no specific provision to maintain fixed asset register. However it is form part of Books of accounts.
As per Caro requirement its compulsary so if CARO is not applicable on your business then its not mandatory.

but every entity has to maintain its fixed asset register details given in the caro 2004.

Fixed asset register format
the fixed asset register must contain:-
date of acquisition
nature of asset (plt & mach etc)
cost recorded in books of account
location
code nos. attached to assets if any
these are illustrative and not exhaustive list of particulars
I need a fixed asset register format. Can any one provide me.

Fixed assets register is required as per sec 209 of the companies act 1956. This register is available in major stationery shops
