presentation is basically just about presenting your financial statements as to how to structure them, minimum requirement for the content. it is basically the need to present your statement of financial position, statement of profit & loss, other comprehensive income, statement of changes in equity and statement of cash flows

Disclosure is basically the need to include important notes regarding a companys assets, liabilities etc on financial statements. Disclosures are more specific aspects about your business that need to presented, failing which, is a wrongful deed. Disclosure can be made in the footnotes , also in the income statement etc.

simply

presentation tells - what is there

disclosure tells - what should be there.
