Filling of return under rule 10 of fixed deposit rules

Dear Friends,

Can anybody provide me format for filling return under rule 10 of fixed deposit rules.

My another query is, in the above form, we have to fill some data at serial no 2(a) & 2(b) of Part 3. SO while providing this data, we have to consider figures i.e. outstanding as on 31st March or money received during the year.

Learnate members pl clarify on this point.
