Fill up Section 80 G

I made some donation on PM Cares fund ( Covid) and while filing my income tax return I have to fill up a form which requires the following details but from where I should get it.

Donation entitled for 100% deduction without qualifying limit:- 

The details are :- 

1. Name of Donee

2.Pan of Donee

3.Address of Donee

4.Pin Code 

and so on. 

Can anyone tell me from where I can get all these details.
