In my may '11 result i got exemption in accounts paper only (PCC). For filling nov '11 examination form i have to submit any documents other than mentioning in the form? Please clarify.

No.. not require any document because while filing your Online form / OMR sheet, there is a column in which you will write your previous exam exemption detail... there is no necessity to attach any document in regard to the exemtion..
