One of my colleague forgot to mention article ship completion date on examination form.. Where we need to give declaration that the articleship will end within 6 months.
Do he need to correct that or is it OK?

Dont worry as now u cannot change your form but while shubmiting the form attach a application form for missing entry of articleship completion date
