ERROR IN SELF SERVICE PORTAL WHILE CONVERTING OLD TO NEW CA FINAL

I was filling CA Final old to New Conversion form on 21.01.2022 but suddenly my Laptop Restarted in the middle of filling the form.

After restarting the laptop I again logged in to the ICAI SSP portal to continue to fill the Conversion form but unfortunately I did not find the Conversion form which was partially filled. Also, I am not getting a link of filling Conversion form afresh gain. (Screenshot attached for ready reference)

Please guide me how I should proceed further.
