Withdrawal from EPF was made during the year. From where can we get the break-up into (Employer's contribution / Employee Contribution / Interest thereon).
Where to show all these details in ITR,

I don't know where to show in itr.
but u can check all contribution made by er, ee & interest from passbook.

In itr 2 or in itr 3 you have to disclose
the employer contribution with interest accrued on such contribution come under the head salaries
the employee contribution as well as the interest accrued on such contribution will be taxed under The head of income from other sources
if you did not claim Deduction as per 80 c in previous years then no taxation but if there's is interest the it's comes under income from other sources
again if you claim Deduction in previous years then there's will be taxation
and all are tax as per the tax rate from the year which the EPF MADE
