What are the documents required for DSC renewal of a director? Should the documents be self attested?

1.PAN card (compulsory for proof of Identity)....in case of NRIs(passport instead of PAN)
2.Proof of Address(Driving License, Utility bills not older than 3 months etc...)
self attestation is enough....
