Dpt 4 related issue

In filing Form DPT 4 in form GNL 2 which option in serial no 3 should be choosen? I had filed DPT 4 for two companies and for both companies i had chosen Return of Deposits wherein one got approved where as the other got rejected and MCA have asked me to select option others in serial no 3 for filing form DPT 4. In Sr. No. 6 what are the section that we need to state and in serial no 8 what would be date of event.
