If you have separate figures, fill both colums...i.e.. DA in taxable allowances

No i don't have seperate amounts!!

Then fill total amount in salary field.....

ITR name will be as per PAN data base.

So, you will have to apply for name change in PAN Card. Once that changed, it will be fatched in IT fields.

In last of return in Verification declaration 

a blank field is there what to be filled there?? Individual?

"I Name  son/daughter of Name  solemnly declare that to the best of my knowledge and beleief , the information given in return is correct and complete and is in accordance with the provisions of Income tax act 1961. I Further declare that i am making this return in my capacity as       ?          and i am also competent to make this return and verify it. "

If the assessee him/herself verifying the return it should be 'INDIVIDUAL', but in case of any other person ...... he/she can be authorized as ... 'legal representative'
