Doubt - conversion from pcc to ipcc

Dear friends and experts,

 I am doing PCC from Nov 2007 without taking much effort. Just now i came to know that Last PCC exam went on Nov of 2012.. Now what is the Procedure ( Forms ) and Fees i have to pay for conversion? and I have completed Articles 3.5 years and CCT 100 hours.. will these be considered after converting into IPCC and shall i undergo any other computer or communication skill course...
