No need to attach any thing with Form-61.  Just write a normal request letter to the concerned ROC mentioning your CIN, and state that you have complied all the necessary legal obligations since the incorporation and state a reason for your failure to file e-forms since introdcution of mCA-21.  Then regret for the failure and assure that you will file all the needed forms and compliances with in the stipulated 21 days time.  Get this letter printed in the letter head of the Company and being signed by the MD.  Then scan and attach, file form 61.  No other things required.- (Rgds.K..GANESH KARTHI)
