While applying for Service Tax registeration what documents are to be submitted along with ST-1

2) trade licence in name of business name, stating name of proprietor/ address and principal business
3) letter signed by proprietor, stating that he is the sole owner of the business.
4) franchise agreement copy, ( if the courier brand name belong to 3rd party)
