If a partnership firm dissolves:
1) is PAN to be surrendered?
2) ITO has to be intimated ? if YES then in which "form" ?
3) is dissolution deed to be submitted anywhere?

Application must be made for surrender. But I don't know about form.
Closure of business has to be intimated within 15 days to the Assessing ITO in charge. Dissolution of firm = closure.
