You should apply in the following format with PAN Card only :

Applicant name:

First Name: Rathod 

Last Name: Gordhanbhai

Middle Name: Rajubhai

Applicant Father's Name:

First Name: Gordhanbhai

Last Name: Rathod

Middle Name: Karamshibhai

  Apply only with PAN card dont confuse with Voter ID. Address proof is enough. so go ahead with the above format.

Hi!

For ID proof use PAN Card and give details as explained by Senthil.  However PAN card will not have address.

Hence for address proof use telephone bill as it closely resembles the name given in PAN Card.  Don't use Votors ID as the names are not having middle / surname.
