DIN for Foreigner

I think no need 4 any affidavite. U can apply. 

For Applyng DIN you just need a valid ID proof & Residence proof.

 In my view, the passport should be valid enough for issue of DIN.

Any other document, i.e, Driving Licesense in this case  might require notarization from

their respective county.
