We just now opened new organisation so we want to register with professional tax. where we have take registration.

Dear Sir , I am an Accountant in a firm we are paying service tax to the Central Govt . We are not registered in commercial tax Department in Karnataka because we are doing service business pay service tax . I want to know about following details. 1. we want to get registration Certificate in Professional tax department. which form to fill? 2.what are the documents required for this regard? 3.Are you mail me soft copy of professional tax registration form for karnatka for an employer.
