suppose I render services in may 2015 & invoice us raised in aug 2015 & income for the same is received in dec 2015. then services should be charged at which rate?

12% and EC, SHEC would also be levied.

Service tax shall be chargeable at the rate of 14%. Since you have neither raised any invoice nor you have received payment, the point of taxation shall be the point of taxation shall be date of payment or issuing of invoice, whichever is earlier, which in your case is August.
