Stipend received in cash

@ nikhil i am also facing the same situation...please answer how did you deal with the same...i too have not filled the bank details in form 108 and received  a letter from ICAI as u have received with refrence to complaince with regulation 48

My ca has paid me In account payee cheque but not in monthly basis... They are asking me reason for non payment of stipend in monthly basis what should I say

I worked as a HR Manager in IT Company. Actually i need a format regarding stipend based training . we provide a training and provide a stipend to candidates. But i need one format which shows like a receipt form for these candidate as a record we gave them amount.
