Withdrawal of Amounts from capital gains accounts scheme

Hi My dear professional colleagues

I have a doubt regarding capital gains account scheme. My client has deposited amount in CAGS. Now for withdrawl of amount from Capital gains account scheme the banker is asking for approval of jurisdictional assessing officer.

Is there any prescribed form or format for making an application to the assessing officer for getting his approval.
