In New e-return It is necessary to add Demat A/c if yes then which one headers nsdl /cdsl Who is Participant? ie. Demat A/c operator?

No. Depository participant means broker.

It is upto you. The ITR form which CBDT notified does not make disclosure of demat account mandatory
