There is no prescribed form for making an application/petition requesting the CIT(A) for condonation of delay in filing the appeal.  It depends upon the facts and circumstances of each case.  Any how I am herewith attaching a proforma.

The affidavit may be drafted basing on the facts. 

This letter may be modified as per the circumstances and status of the appellant i.e individual, firm or company etc.,

I have a client who needs to file an appeal with CIT(A) against 143(3) order, but the time limit has elapsed.  I would like to know, whether your application referred in your message was filed and accepted by the CIT(A).  If so, could you please share that format and the related case laws with me.  Also, I would like to know, if there are any citations where the delay has been condoned by the CIT(A) himself.  If so could you please refer to me. Appreciate your immediate response, as you know, I am running out of time.

hello sir/madam

I have not file ITR for the A.Y. 2010-11 & 2011-12 and want to claim for refund against TDS deducted for the said year. so kindly give me a format of letter of condonation for delay in filling return before CIT accourding to circular no. 09/2015 of Income Tax.
