as far as i kno its st-10

u need to file executive n professional marksheets accordingly

for clarifications better u call institute itself

can we do the training under brothers/ sisters??

If you want to obtain Modified Training Structure then you have to fill SWITCH OVER FORM and Form ST-10 as well.

Switch over form declares that you are swichting to modified structure from old structure then your training will be only 1 year.

i hv some doubts regarding the sequence and exact procedure

1) fill the modified training switch over form and email it

2 ) fill the st-10 form and email it

3) the institute sends me a sponsorship letter

4) enter into contract and email the scanned copy

For getting sponcership letter from ICSI, you have to mail following docs:-

a. Switch over form

b. form ST-10

c. appointment letter or contract

d. scanned copy of marksheets of all stages

then you will receive your sponcership letter within few days.

do i have to first enter a contract and only then shall i receive the sponsorship letter ??

also i know a pcs under whom i have to work  , so should i first enter into contract with her ??

Yes, first you have entered in contract with your employer (PCS) then you will get sponcership letter from iCSI because we have to mail our contract or appointment letter to ICSI, its a proof that we are undergoing in training. 

So you have to mail your contract to iCSI.
