How to raise condonation request to file itr after due date?

CIRCULAR 9/2015 [F.NO.312/22/2015-OT], DATED 9-6-2015
if you have any unclaimed refund amount due to nonfiling of ROI, then you can file ROI for that a.y (but not beyond 6 years) and apply before jurisdictional CIT for condonation
Pls tell me the process to apply condonation?
need to submit a written request to the juridictional CIT. Thats all.
