Please give me format of complaint letter to the IT authorities against a professional who is doing lot of Tax evasion both GST & IT.

You will not find readymade format.
You need only blank paper and drafting skill ..
few points you must put into letter:
- Your personal details
- Autority to whom you are complaining
- Person against whom you are complaining
- subject matter
- your complaint
- resolutions you are expecting
