Quick Summary
To claim the deduction under Section 80U for disability, simply having a medical certificate is not enough. You must file Form 10-IA along with your Income Tax Return (ITR). While some older certificate formats from specific notifications are also accepted, any other medical certificate will not be considered valid for claiming this deduction.

While filing ITR, it's mandatory to file form 10ia to claim deduction under section 80U
I had physically handicap certificate and standard format of medical issued by medical authorities it's enough ?
No its not enough u have to file 10ia for claiming deduction because certificate is only documentary evidence in support of form 10ia

certificate is enough to claim deduction u/s 80 U

FLAT deduction shall be allowed irrespective of actual amount incurred

For claiming deduction u/s 80U, ONLY Form 10-IA or a certificate in the format as prescribed vide notifications No. 16-18/97-NI.1, dated 1-6-2001 and 18-02-2002 are required to be filed alongwith the ITR. The certificates mentioned above are generally similar to Form 10-IA but require an attested photograph showing the disability... any other medical certificate is not admissible.
