Change in exam form

Dear Friends,

I have received Exam form of PCC, In that only OMR form is there . Previously there wrere one OMR form and one other form in which Passing details of CPT,ITT and Other details of Articleship were there.

My doubt is where there is change in this time or I have received incomplete form.
