Ca membership form
Do we have to submit form 6 for COP and form 117 for selection of trade/firm name along with form 2 i.e. enrollment as a member. Or the two columns in form 2 will server the purpose for COP and firm name registration.

No need to submit form Form 6 or Form 117. First of all submit Form 2 along with ID card application form. Based on Form 2 Institute will grant you Membership, COP and approve the firm name.
Once firm name is approved then you have to file the Form 18 for the registration of firm.
Yes fully agree with Prateek's reply, Form 2 suffices and fulfils the requirements of Membership, Holding of Certificate of Practice and seeking approval for use of Trade / Firm name.
Along with Form 2, you must also download Identity card from ICAI website.
