- in which name condonation letter will be made

dd is made in favour of The Secretary The Institute of Chartered Accountants of India
@ anshu: no need to calculate the exact no of leaves. fill approximately after discussing with your employer.

i dont know what is this condonation letter.
address will be same where you deposit other forms
allowed leaves are 1/6th of actual period served.
