Can anyone tell me how to download my articleship completion letter from ICAI website

But I'm unable to get my letter it's showing error
fill the following details
(1) Students
(2) Articleship completion letter
(3) students reg no
(4) Date of birth
(5) code given in website

Can you please tell me that in form 108 in the 1st page it is given that the articles were duly registered with the council of ICAI.. Registration No.. and year. Which year to be mentioned in the form?? 1. When joined Articleship or 2. When 1st time got registered with the Institute.?
