In a private ltd company 2out of 3 directors were Disqualified and one director if left inthe BOD. can you please tell me the procedure to appoint directors in this case and forms to be filed with Roc.

You may take the benefit of Condonation of delay scheme 2018 which is valid till March 2018. File all the pending forms and apply for condonation.
yes, but those directors were Disqualified in other 1 company which is striked off and won't be revived.

please go through the section 174 of Companies Act, 2013. It will solve your problem.
