I have find some problem for din apllication.
when i contact mca wia emial,there mr shukla have reply me for rejectoin of my din apllication.
he give me reson for it is this
Checked the application. Documents are not enclosed with the application and application is not attested.
So what is proble you think there?
it is nessacesory to be attested apllication too?
if yes where in apllication i need to be stamped and signature of attesting authority?
din applicatin also attested along with proof.

But i dont understand,authorisd person can where sign and stapm in din aplication,i means which fild?
And Photo attesting means,Photo affix in din form is need to be attested?
The Photo pasted on DIN application should be cross signed by you and attested by practicing CS/CS/ICWA or any gazetted officer. His signature, stamp and membership no. should hv mentioned on it.
Secondly, u mst attach an Address-ID proof with that application. And fill the no. of that id proof in the blanks provided on the front page of application (voter id no., passport no., as the case may be). Each n every information u fill must be same as the proof u r attaching.

and my telephone bill shaw my name below
Rathod Rajubhai Gordhanbhai
So how can i filled my name in form as first,last and midlle name?
and how can i filled my father name?
I told u that earlier also, only attach voter id . No need to attach any other document, other wise u will be confused and MCA officer will confuse n they will raise another query.
Voter ID is a common proof for ID and residence proof.
