Aiticleship

hi friends i am from mumbai.........my airticleship is completed i haver taken signature on form no 108 and is there any other document with form no 108.

and i am submitting my airticleship completion form late by 1 year how much i have to pay as penalty and in which form cash or dd. if dd on which name it will be.........pls urgent reply
