Dear Respected members,

i need a format of appointment form for trainees/internees.

I also searched it on internet but not found better format.

Actually we are going to hire some Internees/trainees for the 1st time that is why I need it.

Further, someone told me that this form will keep with us in our record after filing information by trainees/internees because of safety reasons so that they can't misuse it against us but we have to issue them a certificate after completion of their training. Please let me know; is this practice being followed in corporate sectors.

Further, also thanks for your response against query about keeping filled form in record, please let me know; should we give copy of said filled form to trainee/internee or it will only be with us after filling by trainee/internee ?

Furthermore, please also let me know; should we handover this form to trainee/internee for filling or should it be filled by us according to provided information by trainee/employee ?

Yes, keep 2 copy of the form if possible or make photo copies and to be filled by trainee and verify the information by authorities.

Actually, i wanted to know that should we give this form to trainee/internee same like we give appointment letter to employees for prove that they are employee in our organization ? or we should keep this form after filling the details by them in our record only and shouldn't give its filled copy to internee/or trainee because they don't have a status of employee.
