Is there any requirement to take precustomized package of preventive health check from hospital/lab centre or self selected preventive test is also eligible for deduction u/s-80D? Please answer with reference if any.

Income tax department has not specified any such restriction for this purpose.
In case of salaried employee expenses incurred on such check ups should not be allowed as medical reimbursement u/s 17. (Just sharing, not relevant here)
