Dear,
You can speak to the stock exchange from which u want to delist the shares; they will guide you and provide you the checklist of the documents that are required to be submitted and compliances that are to be done by the company in this regard.
Iam attaching herewith a manual on delisting from BSE, which may be useful to you.
