Vat audit for financial year 2011-12

Hello,

1) Where can I download the latest Form 704? I can find one at https://mahavat.gov.in/Mahavat/GetDataAction?option=Electronic%20Forms but want to confirm whether I can use the same for FY 2011-12 also.

2) Is it mandatory to first get accounts audited under Income Tax Act(Sec 44AB) and only then file VAT audit report? Or can vice-versa also be done? I don't so because in VAT audit report we've got to mention the date of Income Tax Audit also...
