To get form no.108

Hi I have completed my CA in 2006, but not able to get membership due to non submission of CA articleship completion certificate /form no. 108. I have completed 2 years with one firm and 1 year with another firm. I have got form no. 109 for first two years and not submitted 108 form for another year. But I can see from CA article status I have completed 3 years articleship. But 2nd firm  where I have completed 1 year articleship that firm is not practicing now. I am not able to get Form 108 from that firm. I tried my level best but i am not getting. But I want to enroll as a member, but not able to do it due to not able to get last one year form 108. Please advise me what is the solution for this ?
