Saving bank a/c : joint a/c operation

saving bank account opened name of panchyat with mode of operation , sarpanch and secreatry jointly.

Both signed the withdrawal form on front side as drawers, both side on reverse without any receipt/endorsement/authority to any. The withdrawal is presented at the bank's counter by secretary alone and received cash alone and signed on reverse. Whether this payment is correct/valid? If sarpanch who did not come the branch after 3 years raise issue that fund is misutilised and he is not known what happened, can do? Is any mistake from bank side? It is compulsory to come office all join a/c holders and receive money jointly. It is not third party payment , can one can receive cash on behalf of both?

its not compulsory that authorised signatories shd come to branch personally... if that was the case..what is the use of cheque books??, and even.. their signatures are enough on the frontside of withdrawal form... and only the person receiving amount has to sign on the backside.., So, I dont think, bank did any mistake here, just they have to match and get satisfied with signature specimen cards..,
you said its withdrawal form.. is it a cheque or a withdrawal slip given at bank...
if its withdrawal slip, then both parties' presence is must...
