you need to mention if you're issuing RCM under the Invoice date and number and finally the RCM amount (if it is CGST & SGST or IGST) after Net Amount.
Heading of Invoice--- Bill of Supply
Your Details--- like name,Address,GSTIN,Mob No.etc,
Date & Bill No.
Name of Dealear & address
Item Details:

So there is no need to mention Seperate GST rate in invoice. Under it Amt of invoice is treated as inclusive of GST.
RCM Invoice we raised July.. we can taken ITC create in same month or next month.
for example if you raise the RCM for June month while filing before July 10th then you can take it on July month while filing before Aug 10th.
