Procedure to apply for cop

Dear Friends,

I want to apply for COP. I have resigned from my present company. I want to know that now should i fill up the Form 6 as application. And how much should i pay for same bcz for this year i have paid annual fees as Associate Member without COP of Rs.800/-. Now how much should i pay for same the difference amount or the whole amount of Rs.2000/-.
