Can't get your question. Simple Procedure!!
1. Complete your articleship
2. Fill Form 108
3. Get it signed from your sir
4. Send it via post to ICAI
No need to attach any other document.

U can send registered post to your nearest ICAI office
thank u everyone i was totally confused with different opinions now i m clear regarding form 108
