Father Name 

DOB

Registered PAN address.

E mail will be received at the mail you registered first. (from which you activated link)

and if it is not working 

the last option is to call on banglore call centre no.

The above answer is correct till the extent you are having access of the registered email I'd. Because I.T.Dept sends email at your registered email I'd. If you don't even know the registered email I'd, then only one option is left. You have to apply for a Digital Signature of the assessee. And then you can reset the password and file you I.t.return.
