You need to reconcile it with the reports income tax department has received based on your PAN and if there is any mis match it requires clarification.

First gather all documents concerning income and match it with retruns filed, you will have to prepare latter citing any differences. remember even if financial organiations do not update the 26AS, its your duty to reflect the correct income in ITR.
