Even though the firm who is getting appointed is a proprietorship firm then also the letter should be addressed as plural like (we, us) ???

Previous auditor's NOC is a requirement stated in Professional ethics of the Chartered Accountants Act 1949. There is no specific format for NOC per se. 

The format attached by CA Sonia above is also correct.

whether NOC applicable to Audit fees only or in case any other professional fees is outstanding then in that case NOC is not required.?

Whether it will be considered as professional misconduct, in cases where NOC is not obtained from the previous auditor
