Mvat registration

Even if a service provider's (facilitator of surrogate mothers) turnover of purchases (medicines and other materials) crosses the specified limit under the MVAT Act, he is not liable to get registration under the MVAT. Please correct me if I am wrong.

He is liable to get registration.

But he is a service provider and has obtained service tax registration, than too ? Please reply...

But he is selling products too which is partially taxable under MVAT.

He is not selling them, his patients consume the medicines and he doesn't take money for that separately, i.e. his fees is all inclusive (treatment, consultancy and medicine consumption)
